SSS E1 Form (Personal Record) 2026: How to Download, Fill Out and Get a Copy

If you are about to start your first job in the Philippines, or you are registering as a self-employed member, OFW or non-working spouse, you will come across the SSS E1 Form. Its official name is “Personal Record for Issuance of SS Number” (form code COV-01214). It is the record SSS uses to register you as a member and issue your Social Security (SS) number.

What is the SSS E1 form?

The SSS E1 form is used to register an individual as a member of the Social Security System (SSS) and give them a unique SS number. On the form itself, it is titled “Personal Record for Issuance of SS Number”. The version currently on the SSS website is COV-01214 (09-2015). On the SSS forms page, it is listed as “Personal Data Record”.

It is used by people applying for an SS number as a pre-employment requirement, and by those applying for membership as self-employed persons, Overseas Filipino Workers (OFWs) or non-working spouses.

Since July 15, 2020, SSS has enforced mandatory online registration. In practice, most new members now get their E-1 electronically: the system generates it with your new SS number at the end of the online application (see below).

Purpose of the SSS E1 form

The main purpose of the SSS E1 form is to register you as an SSS member. SSS is the government agency that provides social security benefits to private-sector workers, including sickness, maternity, disability, retirement, death, funeral and unemployment benefits, as well as member loans.

Once you are registered and have paid the required contributions, you can qualify for these benefits. The E-1 also records your dependents and beneficiaries, so it is important that the information on it is correct.

Who needs to fill out the SSS E1 form?

Anyone who does not have an SS number yet needs to register with SSS. SSS requires online registration for prospective employees, self-employed persons, non-working spouses and OFWs.

Household workers (kasambahay) have a separate “Kasambahay Unified Registration Form”, which is listed on the same SSS forms page.

If you already have an SS number, do not register again. Your SS number is meant to last your whole life.

How to Get Your E-1 Through Online Registration

For new members, online registration is the main way to get an SS number. The SSS “Become an SSS Member” page describes these steps:

1. Go to www.sss.gov.ph/become-an-sss-member and open “Apply for an SS Number online”. You can also find it on the SSS homepage. Read the procedures, then click Start.
2. Complete all required fields in the Registrant Record Verification, complete the security check and submit.
3. Open the link sent to your email. It is valid for five (5) calendar days only. If it expires, you have to submit a new application.
4. Enter your basic information, contact details and home address, and choose your purpose of registration. Then add your civil status, place of birth and beneficiaries (father, spouse, children or others, if applicable).
5. Choose your My.SSS user ID and enter your height and weight for the Personal Record. Review your entries and click “Generate SS Number”.
6. Upload a supporting document: a readable JPEG or PDF file, up to 3MB.
7. Check your email for the confirmation and its attachments: the SS Number Slip, the Transaction Number Slip and the E-1/E-6 Form.
8. Use the activation link in the email to create your My.SSS password. The link is valid for 30 calendar days only. After that, you need to register in My.SSS using the Transaction Number shown on your E-1. If you later cannot log in, see our guide on what to do when your SSS account is locked.

Important: once your SS number has been generated, you have to go to an SSS branch to correct any wrong information. Check every entry carefully before you click “Generate SS Number”.

How to Download the Blank SSS E1 Form Online

You can still download the blank, printable E1 form from the SSS website for free, for example if a branch asks you to fill out a paper copy. No registration is needed to download it.

Step 1: Open the SSS Website and Click “Download forms and applications”

Go to the official SSS website at www.sss.gov.ph. Scroll down the homepage to the Bulletin Board section. Under “Need something else?”, click “Download forms and applications”.

Step 2: Choose “Personal Data Record” under Member Forms

You will land on the “Download forms and electronic applications” page. Under Printable Forms, find the “Registration and Membership” section and, under “Member Forms”, click “Personal Data Record”. This opens the E1 form as a PDF.

Step 3: Download and Print the SSS E1 Form

Save the PDF to your device or print it straight from your browser’s PDF viewer. You can also open the file directly: E-1 Personal Record (PDF, sss.gov.ph). The file has three pages: the form itself, the instructions and list of documentary requirements, and an “Additional Sheet for Dependent(s)/Beneficiary(ies)”.

The form says that if you download it from the internet, you should fill it out in two (2) copies. It also says the form may be reproduced and is not for sale.

Instructions to fill out the SSS E1 Form

1. Fill out the form and submit it to the nearest SSS branch office together with the required documents.
2. Fill out only the parts that apply to you:
   • Parts I-A, B and D: if you are applying for an SS number as a pre-employment requirement.
   • Parts I-A, B, C and D: if you are applying for Self-Employed, Overseas Filipino Worker (OFW) or Non-Working Spouse membership.
3. If you need more space in Part I-B “Dependent(s)/Beneficiary/ies”, use the “Additional Sheet for Dependent(s)/Beneficiary/(ies)”.
4. Always write “N/A” or “Not Applicable” if a field does not apply to you.
5. Print all information in CAPITAL LETTERS and use black ink only.
6. If you downloaded the form from the internet, fill it out in two (2) copies.

Important Reminders

1. New registrants over sixty (60) years old who are not surviving spouse pensioners or guardians of a pensioner cannot apply for an SS number.
2. Your SS number is your lifetime number. You should not have more than one SS number.
3. These documents must be originals or certified true copies issued by the City or Municipal Civil Registrar or the Philippine Statistics Authority/National Statistics Office:
   • Birth Certificate
   • Marriage Contract or Marriage Certificate
   • Death Certificate (if applicable)
4. ID cards and documents issued by a foreign government are accepted if they come with an English translation.
5. When you submit photocopies of IDs or documents, always bring the originals or certified true copies too.

What documents do I need for the SSS E1 Form?

According to the list of documentary requirements printed on the form, the main document is your Birth Certificate. If you do not have one, any of the following is accepted:

• Baptismal Certificate or its equivalent
• Driver’s License
• Passport
• Professional Regulation Commission (PRC) card
• Seaman’s Book (Seafarer’s Identification and Record Book)

If you have none of these, you can submit any two (2) documents from the longer list on the form (for example, a postal ID, school ID, TIN card, voter’s ID or PhilHealth ID). Both must show your correct name, and at least one must show your date of birth.

Additional supporting documents depend on your civil status. For example, married registrants submit a marriage contract or certificate, and those reporting children submit their birth or baptismal certificates. OFWs enrolling in the Flexi-Fund Program need a valid Overseas Employment Certificate (OEC) or e-receipt.

What are the common mistakes to avoid when filling out the SSS E1 Form?

Filling out the SSS E1 Form correctly helps you avoid delays and later corrections. Watch out for these common mistakes:

1. Wrong personal information, especially your name and date of birth. These should match your birth certificate.
2. Leaving fields blank instead of writing “N/A”
3. Filling out a part that does not apply to you (for example, Part I-C if you are only applying for a pre-employment SS number)
4. Forgetting to list your dependents and beneficiaries
5. Not signing the form or leaving out the fingerprints, or submitting it without the required documents
6. Applying for a second SS number when you already have one

How long does it take to get an SS number and E-1?

If you register online, there is no long wait. Your SS number is generated at the end of the online application. A confirmation email follows, with your SS Number Slip, Transaction Number Slip and E-1/E-6 Form attached.

Save the confirmation email and your E-1 file in a safe place, and print a few copies. You will need the Transaction Number on your E-1 if you have to register in My.SSS after the 30-day activation link expires.

Lost Your E-1? How to Get a Copy

If you registered online, first search your email for the SSS confirmation message. The E-1 was sent to you as an attachment.

SSS E1 Form FAQs

Do I need to submit an SSS E1 Form every year?

No. You register only once, and your SS number is for life. If your details change later (for example, your civil status or beneficiaries), update your record using the Member’s Data Change Request form (E-4), which is listed on the same SSS forms page.

What do I need to bring when I submit an SSS E1 Form?

Bring your birth certificate or, if you do not have one, an accepted alternative such as a baptismal certificate, driver’s license, passport, PRC card or Seaman’s Book.
